Our Process

Pre-Assessment

Your pre-assessment form arrives via secure link for completion no less than 72 hours prior to your appointment.

Holistic Clinical Evaluation with our MD – 1 hour

The goal of a holistic clinical evaluation is to gain a comprehensive understanding of your health status, identify potential underlying causes of health issues, and develop a personalized treatment plan that addresses your unique needs and circumstances.

Our MD will use a variety of methods to gather information about your health. This can include conducting a comprehensive medical history interview, performing physical examinations, ordering laboratory tests, and considering factors such as your lifestyle, diet, stress levels, social support, and emotional well-being.

Meet with our Wellness Specialist

Our MD will provide his recommended plan of care at this time and our wellness specialist will review pricing. For your convenience, you may prefer a virtual. If so, we will provide a secure telemedicine link through your patient portal.

Lab Work

Peptides are naturally occurring in the body and carry little side effects. However, these are still prescription medications. If prescribed incorrectly they may increase health issues, leave you feeling worse, or result in little to no improvement of symptoms.

A complete blood panel, required to evaluate major body systems, inflammatory markers, and hormones, ensures your safety and success. Lab Corp, ZRT laboratories or via Genova labs evaluates all blood work.

Our medical staff will collect all required bloodwork in the office. To prepare for your blood draw appointment:

  • Fast 10 hours prior to appointment

  • Avoid caffeine and green tea the day of your appointment.

  • Avoid exercise the day of your appointment.

  • Remain hydrated 72 hours prior to your appointment.

All weight loss patients must complete In Mode body scans, weight intake and measurements in the office prior to receiving medications.

Lab work results arrive within 7-10 business days. Our staff then uploads your labs to your patient portal and promptly contacts you to schedule a 45-minute telemedicine appointment with our to review your labs in detail.

Peptides require a systematic approach. We prescribe your individualized peptide program based on your symptoms, labs, and individualized health goals.

Prescriptions

Forma Medical sends all prescriptions to our select, premiere compound pharmacies. No medications are provided onsite or in prefilled syringes.

All payments are made directly to your prescribing pharmacy.

There are no hidden fees or up charges by our office for your prescription medications.

Our staff has gone through extensive measures to identify compounding pharmacies with the highest standards of quality. We vetted each compounding pharmacy for efficacy and compound patents and selected those pharmacies which ensure the highest quality compounds.

Physician Led Peptide Treatment Benefits
